Young Peoples Project
The UnitingCare Supported Living Young People’s Project (SLYPP) has been developed to work intensively with clients with permanent disabilities and complex needs who have a relationship with, or have been identified to be at risk of entering the Criminal Justice System.
Based on intensive Case Management and Life Coaching Frameworks, the Project offers intensive short to medium term case management service.
Case management provides intervention and advice to people with disabilities, their families and carers who are experiencing difficulties with their current support arrangements or who are not in receipt of any services.
The intensive case management service is designed to address the needs of young people who are at risk.
The service aims to assist young people with disabilities to:
- Positively engage client participation
- Provide early intervention strategies to reduce the likelihood of offending or re-offending behaviours
- Maximize opportunities for an enhanced quality of life
- Enhance the clients capacity to live independently within the community and to make informed decisions and positive life choices
- Set clear goals and achievable outcomes.
- Access, develop and maintain links with local community services.
The service works collaboratively following a client-centered approach to guide and facilitate projected outcomes for each client.
Case Managers will:
- Advocate on behalf of clients and promote their self-advocacy skills.
- Communicate and liaise with a wide range of service networks and cross agency cooperation to maximize positive outcomes for individuals.
- Conduct ongoing reviews, assessments, and service evaluation to monitor the effectiveness of service delivery.
- Lobby and improve the quality of service provision to better meet the needs of this client group.
- Engage with families and significant support networks to meet individual needs.
How can the SLYPP Case Management Program help you?
A SLYPP case manager can help to identify needs, and formulate realistic, meaningful and achievable goals for future development. This can include, but is not limited to support and goal setting with:
- Accommodation
- Employment
- Independent living skills
- Leisure and recreation
- Medical and health issues
- Linking of services
- Advocacy
- Building positive support networks
Who is eligible?
Target group for the Project are those who:
- Have a permanent disability with complex needs
- Are aged between 12 – 25 years
- Live in Canterbury, Leichhardt, Ashfield, Burwood, Strathfield, Canada Bay or Marrickville Local Government Areas (LGA)
- At risk of entry into the Criminal Justice system/Juvenile Justice system.
Priority of access to case management services is given to those who:
- Lack of formal and informal support networks
- No access to appropriate day activities, e.g. have been excluded from an educational setting.
- Received more than 1 official caution from Police.
