Case Management
UnitingCare Supported Living Case Management provides a free service which aims to assist people with disabilities and their carers to:
- Set and achieve goals
- Access local services
- Maximise their independence
- Establish and maintain links to relevant community supports and services.
Our case managers achieve this by working closely with service users and their families to meet their individual need. The service provides short to medium term support, usually a period of 6 to 18 months.
UnitingCare Supported Living offers a specialist case management service UnitingCare Supported Living Young People’s Project (SLYPP) to youth at risk of entering the criminal justice system. For further information about this service click here
UnitingCare Supported Living employs case managers from a variety of CALD backgrounds and will engage an interpreter service to cater for non-English speaking clients, at no cost to the client.
UnitingCare Supported Living Case Management service also employs an Indigenous Australian worker.
How can a Case Manager help you?
A case manager will work with you to assist you in identifying your goals and needs, and support you to achieve these. The plan we develop with you is called an Individual Service Plan (ISP).
Your goals may include one or more of the following:
- Employment
- Living independently
- Leisure and recreation
- Accommodation
- Transport
- Medical and health issues
- Linking to services e.g. respite, multicultural or home maintenance services
- Advocacy
- Building social supports
UnitingCare Supported Living case managers will ensure flexible service delivery which focuses on client choice.
Who is eligible?
UnitingCare Supported Living provides case management services to people with a permanent disability living in the following LGA regions:
Inner West:
Canterbury, Leichhardt, Ashfield, Burwood, Strathfield, Canada Bay, Marrickville.
Case management is offered to people between the age of 0-65 years in the Inner West.
South West:
Liverpool, Fairfield, Bankstown.
Case management is offered to people between the ages of 18-65 years in the South West.
South East:
Sutherland, Kogarah, Hurstville, Rockdale.
Case management is offered to people between the ages of 18-65 years in the South East.
Priority of access to the case management service is given to those individuals:
- With ageing carers (65+ years)
- Who are receiving no other services
- Who are in crisis or about to be in crisis
Who can make a referral?
A referral to the case management service can be made by any potential client, a family member, friend, health professional or other service providers.
As UnitingCare Supported Living is committed to providing a client focused service, consent must be obtained before making a referral to our case management service.
What next?
All referrals are discussed at fortnightly referral meetings, and prioritised according to our eligibility criteria. If an individual is eligible and there are vacancies within the case management service, an assessment will follow. Should appropriate goals be identified from the assessment, service is expected to begin following the next referral meeting.
If it is determined at assessment that UnitingCare Supported Living Case Management service is not required our team will provide information regarding other services which may be able to assist.
If an individual is eligible for the service and there are no vacancies, they will be placed on a waiting list. The waiting list is regularly reviewed and people may be re-prioritised if their needs change whilst on the waiting list.
