Accommodation Outreach
UnitingCare Supported Living believes that people with disabilities have the right to live full lives in the community and to participate in activities that are meaningful and of their choice. We further believe that people with disabilities are active participants in and make meaningful contributions to the community, and therefore we work to achieve greater community awareness of the opportunities needed to include people with disabilities in everyday activities.
Accommodation Outreach provides support to people with disabilities who live in their own home in the community. The service provides support to people living in the Inner West, Liverpool, Bankstown, Canterbury, Fairfield and South East areas of Sydney.
This support consists of a mix of in home and community support. The level of support varies significantly according to each individual’s needs.
Accommodation Outreach focuses on encouraging each person to access support which enables them to live as they want to, with the same access to employment, housing, income, education and other services, rights and responsibilities as the rest of the population.
Accommodation Outreach recognises that all people have strengths, talents and abilities which are expressed through interdependence with other people. The support provided is needs based in that we provide enough assistance in areas where it is needed without infringing on autonomy by not interfering in matters with which the person needs no support.
Accommodation Outreach recognises the inappropriateness of providing a single model of support and therefore provides a number of individulalised options of support. Central to any of these options would nevertheless be:
Ø Living in one’s own home
Ø Participating in one’s own community
Ø All planning centered around the individual
Sharing Support and Partnerships with Other Services
Accommodation Outreach also recognises the inappropriateness of one service providing support for all aspects of the person’s life. Therefore the service works co operatively with other services that can provide specialist support like after care, behaviour intervention or mental health services.
Partnership with mainstream services, organisations and networks
All services delivered by Supported Living operate in accordance with the principles of access, equity, participation and human rights.
Therefore where a mainstream service exists that can meet the needs of the people we support, we support the person in accessing this service.
The Nature of Support
The practical nature of support provided to each person is identified on their Individual Support Plan and may include, but not limited to the following:
· Accommodation Support
· Living Skills
· Employment
· Emotional and Behavioural Development – Dealing with personal problems.
· Education and Training
· Financial Support
· Health and Well Being
· Personal, Social and Community Connections
· Contingency Planning (On Call and Crisis Support)
Service Access
All vacancies are managed in accordance with the DADHC Metropolitan South Region, Regional Accommodation Placement Committee Policy. Accommodation Outreach notifies DADHC of any vacancies.
